What is the real impact of Cytoreductive Surgery and HIPEC on Patients? Quality of Life - a Unicenter Prospective Study

Resumo Introdução
CRS+HIPEC is the only curative option for peritoneal carcinomatosis. It is a major procedure with high morbi-mortality. QoL significantly decreases over the 3-6months after CRS+HIPEC and increases up to the basal level 6-12 months after that. This data is only based on retrospective studies. No prospective studies were published. Aim: prospectively analyze the impact of CRS+HIPEC in generic QoL.

Prospective unicenter review of all patients submitted to CRS+HIPEC with curative intent between June2023-September2024. All answered 2 inquiries - SF36 and EORTCC30 before CRS+HIPEC (basal QoL), 3, 6 and 12months after surgery. Exclusion criteria: CC>1 and <3months follow-up.

60 patients were admitted to CRS+HIPEC. 11 were excluded. n = 49, 64.4% female, mean age 59?13 years, primary tumor colo-rectal 37.5%, appendix 25% and ovary 25%. Median PCI 10 (IQR 0-28), CC0 91.7%. SF-36 and EORTCQoL domains 3months after surgery were significantly worse than the basal(p<0.01). There are no differences between basal and 6months(p>0.05). However, there is a significant increase on QoL 1year after surgery, in domains such as pain, physical functioning and physical performance(p<0.05).
Resumo Discussão
Despite a tendency towards lower QoL in the first 3months after CRS+HIPEC, QoL increases up to basal levels over the first 6months and even improves baseline at 12months after surgery. Conclusion: CRS+HIPEC has a positive impact on QoL at 6-12months. This information should be used do inform and guide patients.
