Since the first description of primary hyperparathyroidism, the typical clinical presentation of patients has changed dramatically. Although rare, metabolic complications are still the primary manifestation in young adult patients nowadays.

We present a case of a 23-year-old man with a progressively growing mandibular mass, in which the anatomopathology results in a giant cell granuloma. The lesion was then defined as a brown tumor following the diagnosis of primary hyperparathyroidism.
Resumo Discussão
Brown tumors are late end-organ complications of primary hyperparathyroidism. They are currently rare conditions that can be silent and are particularly rare in young people. Although most cases in literature describe severe symptoms, this situation can be mildly symptomatic. They are treated surgically and the majority is due to a single parathyroid adenoma. In conclusion, the diagnosis of hyperparathyroidism in young patients is challenging and requires a high level of suspicion. Sometimes, this diagnosis is made many years after the onset of the disease.
